+ What is the required release form?
Every camp/clinic has a required release form. The form must be signed by both the participant and parent (if participant is under 18 years of age).
+ When is the Athletic Event Release form due?
The release form is due at registration for the camp/clinic.
Individuals are not allowed to participate in the camp/clinic if the release form is not submitted.
